2017 IMMUNIZATION CLINIC - IMPORTANT INFORMATION

 APPOINTMENT NECESSARY

Children's and MMR for College Students 

• For Children 18 years of age and younger who do not have private health insurance covering immunizations and students of any age currently enrolled in or entering in a post-secondary institution which requires the MMR.
• No Fee for children who do not have health insurance covering immunizations or who are enrolled in a private health insurance plan are considered to be "fully insured" and are not eligible for the Vaccine for Children Program. 
• An administration fee will apply for each injection.
• Immunization records must be provided at the appointment.
• College students requiring MMR must provide forms from college indicating need for vaccination.
• All children under age 18 must be accompanied by a parent or "legal" guardian (legal paperwork of such guardianship required).

Adult Clinic

• This clinic is for adults 19 years of age and over.
• There is a fee which is payable at the time of injection, and is non-refundable.
• The Genesee County Health Department is only able to bill certain insurances for limited vaccinations.  All others must be paid by cash or check by client.
• The Genesee County Health Department cannot bill Medicare for these services. 
• Medicaid and other state-funded programs (Child Health Plus, Family Health Plus) will NOT reimburse under these circumstances.
• Please contact your insurance provider for reimbursement information prior to receiving the vaccination at a Genesee County Health Department Clinic.
