CHILDREN’S CLINIC

• No Fee - covered through the NYS Vaccine for Children’s Program for those children who do not have health insurance, which covers immunizations
• For Children 18 years of age and younger and college students requiring the MMR, who do not have health insurance, covering immunizations.
• Immunization records must be provided at the appointment.
• College students requiring MMR must provide forms from college indicating need for vaccination
• All children under age 18 must be accompanied by a parent or “legal” guardian (legal paperwork of such guardianship required)

ADULT CLINIC

• This clinic is for adults 19 years of age or over
• There is a fee which is payable at the time of injection, and is non-refundable.
• Young womdn, ages 19-26 may qualify for the HPV vaccine through a special program offered by Cancer Services Program. Please contact them at (585) 344-5494 for more information.
• The Genesee County Health Department is unable to perform third party billing under any circumstances.
• Medicaid and other state-funded programs (Child Health Plus, Family Health Plus) will NOT reimburse under these circumstances.
• It may be necessary to receive the vaccination from your private physician in order to receive insurance benefits. Please contact your insurance company for reimbursement information prior to receiving the vaccination at a Genesee County Health Department Clinic.
