Anti-Rabies Clinic Pre-Registration Directions
1. PRINT TWO COPIES of each registration form you fill out and bring to clinic.
2. Fill out the owner contact information and pet information in the pre-registration form located at the link below.
3. If you own more than one animal, simply click the “Clear Animal” feature located in the upper left-hand corner of the form. This feature allows you to input the information of another pet, without erasing the owner’s contact information.
