CASEWORKER (PUBLIC HEALTH)

DISTINGUISHING FEATURES OF THE CLASS:
The incumbent in this position provides medically related social work services for children and their families, to assist them with their medical/health related difficulties.  The work is performed under direct supervision.  The caseworker, in consultation with his/her supervisor (program coordinator) formulates and carries out plans to meet an individual’s timed needs.  The incumbent coordinates the plan with other service providers, and monitors his/her participation in the primary health care system.  Home visits and the transportation of individuals may be required.  Does related work, as required.

TYPICAL WORK ACTIVITIES:
Identifies individuals, including children, with Special Health Care Needs by conducting outreach activities and working cooperatively with other service providers, both within the agency and in the community;
Establishes and maintains linkages between the Department of Health and health care providers, such as: hospitals, pediatricians/primary health care practitioners, Child Health Plus providers, etc.....;
Facilitate access to health care coverage to individuals by conducting personal (family) interviews to identify needs for service.  Assistance with enrollment, follow-up and monitor participation in the primary health care system;
Coordinate efforts with primary health care providers to locate and recover children and/or individuals who have been lost from the primary health care system;
Collect, maintain, analyze and submit data and information as specified by the County and State;
May be required to facilitate certain programs within the agency and work closely with other staff personnel in carrying out plans for services;
Drives to family homes as necessary;
Data entry into computer.

FULL PERFORMANCE KNOWLEDGE, SKILLS, ABILITIES AND/OR PERSONAL CHARACTERISTICS: Good knowledge of case management techniques; working knowledge of Federal, State and local health laws and programs as they related to individuals (including children) with special health care needs; working knowledge of available services for clientele; working knowledge of the use of computers and data entry; ability to establish and maintain successful relationships with people; ability to operator a motor vehicle; resourcefulness; skills in organization and communication; good powers of observation and analysis.

The employee must occasionally lift and/or move up to 25 pounds.  Specific vision abilities required by this job include close vision and the ability to adjust focus.

MINIMUM QUALIFICATIONS:  Graduation from high school or possession of an appropriate equivalency diploma recognized by the NYS Department of Education, AND EITHER:

  1. Graduation from a regionally accredited or a New York State registered college or university with a Bachelor's degree in Human Services, Education or Social Work or other related field,
    OR:
  2. Graduation from a regionally accredited or NYS registered college or university with an Associate’s Degree in Human Services or related field AND two (2) years of full-time, paid experience in a human services field where the individual has performed care management duties.

SPECIAL REQUIREMENTS FOR APPOINTMENT AND CONTINUED EMPLOYMENT:  Possession of a valid NYS Operator’s License.
