General Application Procedures

We accept applications for SNAP, Medicaid and Temporary Assistance Monday through Friday from 8:00 AM - 5:00 PM. Temporary Assistance interviews are conducted first come, first serve for applications submitted before 3:00 PM. Individuals submitting Temporary Assistance applications after 3:00 PM will be interviewed that day only if they have a documented emergency (an emergency is homeless, no food, and/or no heat).  All others will be asked to return the following day for a temporary assistance interview. When submitting a Temporary Assistance application please be prepared to wait to be interviewed. We service people as quickly as possible; however, each interview time varies, depending on the needs of the applicant.

You should bring as much documentation as possible when submitting an application. You will be required to verify your identity, residence, household composition, income and household expenses.

If you would like to apply for Medicaid or Child Health Plus and you are not applying for one of the following Medicaid Programs:

Chronic Care (Nursing Home), Community Medicaid for Individuals 65 and Older, One of the Medicaid Premium Payments or Medicaid Buy-In for Working People with Disabilities you may call Lake Plains Community Care Network at 345-6110 for an appointment or go online to www.lakeplains.org

If you are receiving assistance and need to see your worker, you should call 585-344-2587 to make an appointment. If you have an emergency, you can see your worker without an appointment from 10:30 am - 11:30 am only.
