FluSafe timeline



      This timeline includes important campaign dates and directions

        on how to document proof of  immunizations on NYSIIS.


     In order to participate in FluSafe this spreadsheet needs to be completed and

      submitted by December 31, 2013. 


      Changes to NYS Public Health Law 2168 pertaining to NYSIIS 

      effective July 12, 2013 



*FluSafe's Definition of "Employee"

For the purposes of FluSafe, employees include all paid personnel in a health care setting, including licensed in dependant practitioners.  Include employees who work in your facility for 30 days or more from October 1, 2013 - March 31, 2014.*


       Directions on how to obtain both an account on the NYSDOH

     Health Commerce System (HCS) and the New York State

     Immunization Information Registry (NYSIIS). Each individual user

     must obtain both accounts for both in order to access NYSIIS.

       This form allows facilities to track the reasons health care workers decline
