Long term care is offered in many different settings.  They include nursing homes, rehabilitation centers, assisted living, enriched housing, congregate care centers, and family type homes where you live in the facility in which you get your care.  You can also stay in your home/apartment where services are brought to you.  A wide range of care is available.   No matter where you are, you may be able to get medical care, therapies, help with your medication, bathing, dressing, mobility, shopping, and household chores.  Most types of services are offered in more than one setting to let you live freely for as long as you are able to.

 In Your Home

You don’t have to move from your home to get many long term care and services.  Most home health care agencies, homemaker service companies, and physical therapists will offer their services in your home.  You can have changes done to your home to make it easier for you to live there.  For example, you can have bathtub safety bars installed, which can make it easier for you to stay there.

The more care you need, the more it will cost to have services in your home.  The high cost of care in your home is something to think about as you look at this option.  There may be programs or services available for you at no cost or low cost, depending on your individual situation.

If you are able to attend one of these centers for care and services, you can still live in your home instead of moving into a nursing home or other facility.  There may be programs or services available for you at no cost or low cost, depending on your individual situation.

In A Residential Care Setting

Where Can I Find the Best Home, Community-Based Care, Residential, and Nursing Facility Care Services? 

Often, family, friends, and neighbors are the best source of information about home-based care.  They also may be willing to help you with the care and services you need.  Sometimes, you can also get advice by asking your doctor or minister.

Home health agencies, visiting nurse associations, homemaker service agencies, and others can arrange help for you at a cost.  You want to check references to make sure these agencies have good training and services to meet your needs. 

Another way to find the care agency you need is to look in your local newspaper.  You can also find listings of local providers in the yellow pages of your telephone book.  Again, be sure to interview these people and check references about the training and experience of these agencies. 

You may want to move to a residential care setting or a nursing home facility.  Finding a setting that suits your needs and makes you feel comfortable is important.
